Our Podiatry Team Is Ready To Help You
Your visit to our practice, Maple Springs Foot Center, establishes a vital foundation for our relationship with you. During the first visit we will make sure to obtain important background information, your medical history, and give you time to get to know our doctors.
When filling out the “new patient paperwork” please be specific with your medications. Include the name and dosage for each medicine. If providing a list is more convenient for you, please do so. You can even save some time at your first visit by printing out and completing the patient forms in advance of your appointment.
What to bring to your appointment
Patient's Health Insurance Card(s)
Photo ID
Primary Care Doctor's name and telephone number
Referral From Your Primary Care Doctor (If Required By Your Insurance Carrier)
List of current prescriptions and/or over-the-counter medication, including dose and frequency
Information about patient's medical and surgical history
Any Previous X-Rays, MRIs or Test Results Relating To Any Past Foot Health Conditions

We do our best to have patients seen at the time of their appointment, in order for us to continue to do so, please be on time for your appointment. If you choose to fill out the new patient paperwork the day of your visit, please arrive 5-10 minutes prior to your appointment time.
